CR40 Ironbreaker here. RR doesn't matter because I did nothing but test PvE since there was really nobody to fight tonight.
I use an axe and shield and stack for survivability when doing PvE. I pull multiple targets and use Earthshatter (http://www.wardb.com/spell.aspx?id=1387), Shield Sweep (http://www.wardb.com/spell.aspx?id=1361), and Rune-Etched Axe (http://www.wardb.com/spell.aspx?id=1366) to deal AoE damage while focusing on the biggest target with my self-buff attacks and Shield of Reprisal (http://www.wardb.com/spell.aspx?id=1369).

Grudge generation is fine in solo PvE. In fact nothing has changed in terms of solo generation so this is to be expected. However, if I wanted to maintain my Grudge from fight to fight I could not stop to loot or Scavange. The 10 second decay timer combined with the incredible rate of decay requied me to quickly loot what I could and keep moving if I wanted to maintain high Grudge.
I had to grind in Public Quest areas, by and large. No other locations provided me with enough mobs for me to keep my Grudge up from encounter to encounter. Normal "quest" areas had mobs that were too spread out, so I had to move to pull the next mob as the first one died.
Pulling 3-5 even-con mobs worked best. I could get my Grudge up to 100 quickly then start rotating through my AoE attacks. Lots of blocks meant I was taking 15% less damage much of the time thanks to Seasoned Veteran (http://www.wardb.com/spell.aspx?id=1395).
I found a PQ that has critters nearby (CR1 rats). After dropping a bunch of mobs I would Taunt the critter onto me while I looted the corpses. This would prevent my Grudge from dropping, though I couldn't Scavange because getting hit interrupts it.
Due to high avoidance, there were times when I would not get hit for 10 seconds (fighting a single mob, of course) and my Grudge would start to drop in-combat.
My overall grinding speed was noticeably slower, but still tolerable as long as I could pull multiple mobs at a time.
