I hope some of you will find this useful, they are from the notes of a wandering Warrior Priest called Caradoc who dabbles in cultivating and Apothecary in the gods-forsaken lands of Sylvania. As such they will be bias towards the cultivating side.
I shall make the assumption that you have a basic understanding of this art. Otherwise more information can be found here (http://www.warhammeralliance.com/forums/showthread.php?t=89508) from one of our Elvish brothers skilled in the arts of healing. If you need something even more basic - please look over on the WAR ign vault here (http://warhammervault.ign.com/wiki/index.php/Apothecary_101). It even has pictures.
I've tried to illuminate the ingredient so you can see where to aquire it from. Cultivation Butcher and Scavenging.
Cultivating comes with it's own little caveat, with each tier the seeds change names. I've placed the tier to the right of the plant name.
Below the Ingredient I've put the potion names and levels. Don't assume that each main will produce the same potion. It can jump a tier above or below.
Anything I'm unsure of I've put in italics - I would love confirmation of what it is.
Offensive Potions:
Thorn Shield - Dryad Husk
Make things a bit prickly for your opponent
thistle (5), Bristle (10), Quill (20), Thorn (30), Spur(35)
Thrown AoE DoT - Daemon Blood
Just light it and throw it, explodes and burns on impact - nice little dot. Found that beating back daemons around a windmill.
Blast (5), Smoldering Blast (10), Searing Blast (20), Flaming Blast (30), Scorching blast(35)
Single target DOT - Pyre Ivy T1, Scruntleydown T2, Spikewort T3, Dragoncap T4
Usual thing with the water and hey presto, you got a potion to set fire to someone. Its a single target DoT.
Blistering (5), Smoldering (10), Searing (20), Flaming (30), Scorch (35)
cone base AE - Ashberry T1, Redvine T2, Yellow Canny T3, Creep T4
Simular thing - except this is like eating a rather hot halfling pie and doing a dragon impression. Char breath, certainly - firey breath that clears your front arc.

AoE Snare - Ogre Spit
Thrown snare with limited range.

Intelligence - Smedley Cap Mushrooms T1, Twigfester T2, Toody's Hat T3, Jack O' Lantern T4 or Skaven spit
Something those pesky bright wizards love.
Insight (5), Guile (10), Judgement (20), Knowledge (30), Acumen (35)
Wisdom - Grump leaf T1, Hairy Elfcup T2 or Ulric's Lash T3, Spumepetal T4 or Daemon Whiskers
Guaranteed to increase even an Orc's Wisdom.
Ambiton (5), Purpose (10), Will(20), Resolve (30), Determination (35)
Ballistic Skill - Thief nettle T1, Twigbloat T2 or Flea Agaric T3, Cheese Slipper T4 or Feather
I know a few Dwarvish Engineers who love this stuff.

Sure, your'll have the odd Blademaster who'll want this, but recently I seem to be using it more myself as those Heretics seem to be ignoring them and hitting me.

Spirit Resistance - Grave Dirt or Defiled Earth
Something your'll want for those fighting degenerate so called Priests that don't follow Sigmar.

A nice bonus for slaying these horrors - Sigmar be praised!

Corporal Resistance - Special Gravelnuts T1, Special Trotweed T2, Special Old Mans Mane T3, Special Hookberry T4,
I'm still not convinced on the names they use will consistantly produce a resistance potion. Generally if the prefix is a colour or sounds "broken" jagged/Serrated it produces this. Ones that don't sound 'broken' smooth or are about 'movement' make the toughness potions.

Healing Potions:
I haven't put in the potion names and levels as I'm still confused with these. I'm having a debate over here (http://www.warhammeralliance.com/forums/showthread.php?t=140155) about my understanding - and if certain healing potions have bugged itemisation or are working as intented.
Action Points - Dande dragon T1, Peck Grass T2, Sorrel T3, Foolsberry T4, or Fleas
Heat with some water - makes a potion to restore action points. Generally loved by tanks, and a reminder for those tanks that have been hit around the head once too many times - they are the yellow ones.
Heal over Time Elixir (HoT) - Musty Elvish Parsley T1, Probing grottok weed T2,Black Pea T3, Green Grundle T4, wolf blood or Troll Spit
Heals like me when I'm not around. Note that a differently named version of the plant/seed makes a Insta heal potion detailed below. I'm still not convinced on the naming convention though.
Intsa heal Draught - Ticks, Leeches or SPECIAL Elvish Parsley T1 SPECIAL grottok weed T2SPECIAL Black Pea T3, SPECIALGreen Grundle T4
A green looking potion, with an instant effect. Trenchant/Probing is the seed types that produce the insta heals.
Damage Ward potion - Domed Morrweed T1, Skunk Thistle T2, shelled Chev's Needle T3, UNKNOWN T4,
This ones a yellow potion. Smells a bit like a Dward's beard.
Screening (5), Blocking (10), Defending (20), Protecting (30), Guarding (35)
Stablisers
To stablise your potion you will start with vendor brought Water. As soon as your skills become better and the potions you start making more advanced you will need to start using something more powerful. These can be Water Zoic Gore Arboreal Resin.
You also need to be aware that the vial you use is also a stabiliser. My understanding is that you add the level of the vial and stabiliers together to get your total stability. Again there is an appendum here - the level 1 vials and waters actaully count as a level 25 stabiliser for this calculation. Empty stabliser slots reduce the stability. Extra's
Number of Potions Produced - Fusk T1, Bolete T2,Shadow Fungus T3, Clubmoss T4

The above do appear to alter the effects - but its been inconstant, and I'm praying to Sigmar for his guidance here. I've had some experience with squig parts that worked well from a rather darker place in my travels.
